Centered Care LLC  proudly operates Adult Foster Care Homes licensed through the State of Michigan. Centered Care is Joint Commission Accredited for Behavioral Health, Nursing, and Home Care. Our specialty is Spinal Cord Injuries and Brain Injuries.  We are equipped to take care of those who have mobility issues, ostomies, respiratory issues, enteral feedings, neurogenic bowel and bladder, and behavioral issues.

Centered Care's AFCs embrace person-centered planning, family participation, and an interdisciplinary approach to provide continued post-acute rehab in a way that respects the dignity and privacy of the individual.  Our staff of Nurses and aides work together with the patient, physicians, Physical Therapists, Occupational Therapists, and other disciplines that visit our home to reignite passions and help give increased purpose and joy to lives.

Our post-acute rehab AFCs offer various services to assist with the rehabilitation process. These may include physical therapy, occupational therapy, speech therapy, medication management, and assistance with activities of daily living. Additionally, staff members are trained to provide medical monitoring, personalized care plans, and emotional support to residents.

The goal of our post-acute rehab AFCs is to promote independence and assist residents in regaining their functional abilities. With a team of qualified healthcare professionals and rehabilitation specialists, our facilities aim to help individuals regain strength, improve mobility, enhance self-care skills, and manage any chronic conditions they may have. Our program offers a well-equipped gym, therapy rooms, and specialized equipment to facilitate the recovery process. We also offer social and recreational activities to promote a sense of community, mental wellness, and overall wellbeing.
